Florida Licensed Continued Stay Coordinator

Job Summary

To support continued care for participants, the full-time Florida Licensed Continued Stay Coordinator will facilitate eligibility determinations, conduct functional reviews, and collaborate with interdisciplinary teams in a remote environment.

Key responsibilities

• Facilitate Continued Stay Reviews (CSRs) and make referrals for Level of Care (LOC) assessments
• Conduct periodic functional eligibility reassessments and analyze clinical and psychosocial information to determine continued eligibility
• Collaborate with team members to communicate outcomes and ensure compliance with regulatory standards

Required qualifications

• High school diploma or equivalent
• Minimum of two years of experience in eligibility determination, continued stay reviews, or related healthcare roles
• Knowledge of healthcare regulatory frameworks, such as Medicaid and PACE eligibility standards
• Proficiency with electronic health records or case management systems
• Ability to manage multiple cases and prioritize deadlines effectively
