Key responsibilities

• Conduct virtual adult and pediatric wellness visits, health assessments, and medication reviews
• Collaborate with in-home team members to evaluate vital signs and arrange necessary patient services
• Provide comprehensive care coordination, including preventive care interventions and referrals to specialists

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner license in Florida
• Experience performing wellness visits
• Technologically savvy with the ability to use HIPAA secure messaging apps
• Commitment to professionalism, integrity, and high-quality patient care
