Job Summary

A company is looking for a Health Coach (Cardiology).

Key Responsibilities

• Provide patient education on chronic disease management and heart failure
• Establish and monitor patient goals, ensuring engagement and satisfaction
• Collaborate with the clinical team to optimize patient outcomes and participate in team meetings

Required Qualifications and Background

• Health coach certification from an accredited program; bachelor's degree in health sciences preferred
• Experience in a medical office or healthcare environment; cardiology coaching experience is a plus
• Experience with telehealth is advantageous
• Proficiency in electronic health records and Microsoft Office applications is strongly preferred
• Bi-lingual fluency in English and Spanish is highly preferred
