Job Summary

A company is looking for a Health Coach (Cardiology) who must reside near Orlando, FL.

Key Responsibilities

• Provide patient education on chronic disease management, focusing on heart failure
• Establish and monitor patient goals while ensuring excellent patient experience and engagement
• Collaborate with the clinical team to optimize patient outcomes and participate in interdisciplinary meetings

Required Qualifications

• Health coach certification from an accredited program; a bachelor's degree in health sciences preferred
• Experience in a medical office or healthcare environment; cardiology-related coaching experience is a plus
• Familiarity with telehealth practices is beneficial
• Proficiency in electronic health records and computer applications, such as Microsoft Office
• Bi-lingual fluency in English and Spanish is highly preferred
