Job Summary

To support a value-based care model, a full-time Florida Licensed Health Coach will provide education and coaching on heart failure management, collaborate with clinical teams, and enhance patient engagement in a remote work environment.

Key Responsibilities

• Provide education and coaching on heart failure, lifestyle modifications, and self-management techniques
• Collaborate with the clinical care team to support patient care plans and document patient progress
• Respond to patient communications promptly, building strong relationships to ensure a high-quality patient experience

Required Qualifications

• Bachelor's degree in health sciences or a related field preferred
• NBC-HWC (National Board for Health & Wellness Coaching) certification required
• Experience in healthcare, chronic disease management, or patient engagement preferred
• Cardiology or heart failure experience is a plus
• Telehealth or remote patient engagement experience preferred
