Job Summary

To deliver acute, hospital-level medical care directly to patients' homes, the full-time Advanced Care at Home Physician will utilize virtual technology, remote monitoring, and in-person visits while collaborating with a multidisciplinary team.

Key Responsibilities

• Review and accept new hospital admissions or patient transfers into the home care program
• Conduct virtual or in-person rounds to monitor recovery and update treatment plans
• Collaborate with nurses, pharmacists, and medical specialists to ensure seamless treatment coordination

Required Qualifications

• Active medical license (MD/DO) eligible for licensing in PA, NL, DE, MD, and FL
• Clinical experience in managing children with medical complexity and acutely ill patients
• High comfort level using Electronic Medical Records (EMRs) and remote patient monitoring tools
• Ability to make independent clinical decisions in a non-traditional environment
• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
