Job Summary
To ensure the proper delivery of services to active PACE In Home Services Participants, the full-time Florida Licensed In-Home Services Coordinator will manage scheduling, oversee compliance with care plans, and serve as a liaison between clients and caregivers in a remote environment.
Key responsibilities
- Coordinates client schedules and ensures compliance with Individualized Service Plans and Care Plans
- Acts as a liaison between clients, families, and service providers, informing them of available services and scheduling changes
- Maintains client records and oversees adherence to federal, state, and local regulations
Required qualifications
- High School Diploma or GED
- 2 years of related administrative experience
- Experience in gerontology and/or medical/social services is preferred
- 1 year of scheduling or logistics management experience is preferred
- 1 year of experience in the healthcare profession is preferred
