Florida Licensed Lead Care Manager
Job Summary
Overseeing care plans and service management activities, the full-time Florida Licensed Lead Care Manager will work remotely to provide expert guidance on personalized care plans for complex care members while collaborating with the care management team and healthcare providers.
Key responsibilities:
- Oversees and reviews ongoing care plans, collaborating with providers to address member needs
- Provides expert guidance to the care management team to ensure quality control and adherence to policies
- Monitors member status and outcomes, revising care plans as necessary based on identified needs
Required qualifications:
- Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
- 5 - 7 years of related experience in care management
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
- Specialty pharmacy experience is a plus
- Experience in telephonic or digital outreach to assess member needs
