Job Summary
Seeking a compassionate and organized Florida Licensed LPN Coordinator, this fully remote, full-time position will manage post-acute care services for injured workers, collaborating with healthcare providers and case managers to ensure effective care transitions and positive recovery outcomes.
Key responsibilities
- Process referrals for injured workers and obtain necessary clinical information from various sources
- Coordinate care placements and follow up with healthcare facilities to monitor progress and discharge needs
- Maintain consistent communication with clients regarding the status of injured workers and address inquiries professionally
Required qualifications
- Current LPN state license in good standing with the State Board of Nursing
- Minimum of one year of nursing experience
- Knowledge of injured worker's rights and confidentiality of Protected Health Information (PHI)
- Excellent organizational, prioritization, and multi-tasking skills
- Proficient in standard office software and able to learn proprietary systems
