Florida Licensed LPN Coordinator

Job Summary

To support post-acute care services, the full-time Florida Licensed LPN Coordinator will manage referrals for injured workers, coordinate care with healthcare facilities, and serve as the primary contact for providers and case managers in a fully remote capacity.

Key responsibilities

• Process referrals and obtain clinical information for injured workers from various sources
• Coordinate staffing and communication regarding the progress of injured workers with clients and healthcare providers
• Monitor and follow up on care progress, ensuring appropriate discharges and additional services as needed

Required qualifications

• Current LPN state license in good standing with the State Board of Nursing
• Minimum of one (1) year of nursing experience
• Knowledge of injured workers' rights and confidentiality requirements
• Strong understanding of medical terminology and excellent organizational skills
• Proficient in computer skills, including Outlook and Teams
