Job Summary

Working remotely, the Florida Licensed Maternity RN will manage care coordination and patient education for members of the Healthy First Steps Program, focusing on assessing health status, identifying treatment gaps, and facilitating smooth transitions from care environments.

Key responsibilities:

• Making outbound and receiving inbound calls to assess and support members' health needs
• Coordinating care and making referrals to external resources as necessary
• Documenting findings and educating members on disease management and healthy lifestyle changes

Required qualifications:

• Current and unrestricted Registered Nurse license in the state of Florida
• 3+ years of experience in a hospital setting, particularly in OB and/or Maternity care
• 1+ years of experience with MS Word, Teams, Excel, and Outlook
• Must reside in the advertised region
