Job Summary

A company is looking for an ER Follow-up Coordinator.

Key Responsibilities

• Review and evaluate patient care settings using information from the Event Notification System
• Organize and share resources with staff to assist patients in obtaining appropriate care
• Analyze reports and communicate findings to support patient care plans

Required Qualifications

• LPN or Medical Assistant Certified with 1-2 years of related experience or equivalent education
• Knowledge of Electronic Health Record software and Microsoft Office applications
• Ability to interpret safety rules and procedure manuals
• Mathematical skills for basic calculations and data interpretation
• Preferred certification as LPN/MA
