Key Responsibilities

• Coordinate patient enrollment and engagement in the Heart Failure Program
• Manage telehealth schedules and conduct patient intake over the phone
• Provide patient-centric care and maintain patient records while ensuring follow-up and care management

Required Qualifications

• Certified Medical Assistant through a recognized certification exam
• High school diploma or GED
• At least 3 months of experience in a medical office setting preferred
• Strong computer skills and knowledge of medical terminology
• Ability to work remotely and maintain patient privacy
