Job Summary

A company is looking for a Nurse Navigator 1 - Site Disease to work remotely in a full-time capacity.

Key Responsibilities

• Triage new patient appointments and ensure timely scheduling
• Educate patients on treatment plans and support them throughout the care continuum
• Collaborate with a multidisciplinary team to improve patient care and address barriers to treatment

Required Qualifications

• Bachelor's Degree in Nursing
• Registered Nurse Licensing (RN)
• Minimum of 1 year of oncology experience
• Ability to assess patient needs and facilitate appropriate referrals
• Experience with Electronic Medical Record systems
