Job Summary

A company is looking for a Nurse Practitioner Resource for Evening Nights & Weekends (Per Diem).

Key Responsibilities

• Provides fully remote virtual serious illness care management to patients and families across various care settings
• Consults with medical staff and performs virtual assessments to ensure optimal patient care
• Documents interactions in the electronic medical records and participates in quality assurance activities

Required Qualifications, Training, and Education

• Minimum of 3 years of recent clinical experience in hospice, palliative care, or acute inpatient care
• Must hold a Master's or Doctor's degree in Nursing from an accredited Family or Adult Nurse Practitioner Program
• National certification, licensure, and prescriptive authority from the appropriate governing body are required
• Must maintain Basic Life Support certification and obtain Advanced Certification in Hospice and Palliative Nursing within 3 years
• Compact RN license in the home state is required by the date of hire
