Job Summary

A company is looking for a Nurse Practitioner for Evenings, Nights, and Weekends (Per Diem).

Key Responsibilities

• Provides fully remote virtual serious illness care management to patients and families
• Consults with medical staff and performs virtual assessments of patient needs
• Documents interactions in electronic medical records and participates in quality assurance activities

Required Qualifications, Training, and Education

• Minimum of 3 years of recent clinical experience in hospice, palliative care, and/or acute inpatient care
• Masters or Doctor's degree in Nursing from an accredited Family or Adult Nurse Practitioner Program
• National certification, licensure, and prescriptive authority in the home state and additional states
• Basic Life Support certificate and compact RN license in the home state by date of hire
• Advanced Certification in Hospice and Palliative Nursing (ACHPN) must be obtained within 3 years of hire
