Job Summary

A company is looking for a Nurse Practitioner for Evenings, Nights, and Weekends on a part-time basis.

Key Responsibilities

• Provide fully remote virtual serious illness care management to patients and families
• Consult with medical staff and perform virtual assessments of patient needs
• Document interactions in electronic medical records and support discharge planning

Required Qualifications, Training, and Education

• Must have a minimum of 3 years of recent clinical experience in hospice, palliative care, or acute inpatient care
• Must hold a Master's or Doctor's degree in Nursing from an accredited program
• Must have national certification, licensure, and prescriptive authority in the home state and additional states as required
• Must maintain Basic Life Support certification and obtain Advanced Certification in Hospice and Palliative Nursing within 3 years of hire
• Must have a compact RN license in their home state by the date of hire
