Florida Licensed Nurse Practitioner

Job Summary

To support a growing telehealth platform for older adults, the part-time Florida Licensed Nurse Practitioner will partner with patients to create personalized lifestyle care plans, coach them through behavior change, and collaborate with a multidisciplinary team in a fully remote environment.

Key responsibilities

• Assess health and wellness goals of older adults and develop evidence-based lifestyle care plans
• Guide patients in sustainable behavior changes focusing on nutrition, movement, sleep, and stress management
• Collaborate with behavioral health teams and coordinate with patients' existing primary care providers

Required qualifications

• Active Florida Nurse Practitioner license with Full Practice Authority or equivalent qualifications
• Experience or training in lifestyle medicine principles
• Genuine enthusiasm for working with older adults
• Comfortable with telehealth practices and adaptable to a dynamic work environment
• Strong collaborative skills and a mission-driven approach to care
