Job Summary
Providing fully remote virtual serious illness care management, the full-time Florida Licensed Nurse Practitioner will deliver care to patients and families across various service lines, including hospice and palliative care, while working evenings, nights, and weekends.
Key responsibilities
- Conduct virtual assessments and manage care plans for patients and families in the continuum of care
- Collaborate with medical staff and interdisciplinary teams to ensure optimal patient care and continuity
- Document all interactions in the electronic medical records and participate in quality assurance activities
Required qualifications
- Master's or Doctorate degree in Nursing from an accredited Family or Adult Nurse Practitioner Program
- Minimum of 3 years of recent clinical experience in hospice, palliative care, or acute inpatient care
- National certification, licensure, and prescriptive authority in the home state and any additional states where care is provided
- Basic Life Support certification from an approved organization
- Compact RN license in the home state by the date of hire
