Job Summary

A company is looking for a Florida Licensed Occupational Therapist (Care Navigator) - REMOTE.

Key Responsibilities

Provide remote monitoring services to support patients in their home-exercise therapy program
Direct and assist users in therapeutic exercise, neuromuscular re-education, and functional training through a technology platform
Maintain necessary records and provide ongoing feedback to the evaluating clinician on patient progress

Required Qualifications

Active Connecticut OT license
Graduated from an accredited physical therapy program
2+ years of experience in musculoskeletal health is a plus
Tech-savvy, with access to a newer model laptop and smartphone
Experience in behavioral modification or Health Coach Certification is a plus
