Job Summary

A company is looking for an Oncology Registered Nurse Patient Navigator to provide individualized assistance to patients and families in overcoming healthcare system barriers.

Key Responsibilities

• Serve as a liaison between patients, families, and healthcare providers to ensure effective communication
• Monitor patient symptoms and manage high-risk, complex patient care to minimize readmissions
• Provide ongoing education, resources, and referrals to patients and caregivers while promoting a patient-centered care environment

Required Qualifications, Training, and Education

• Registered Nurse multistate or Florida single state licensure required
• Bachelor of Nursing preferred
• Certification as an Oncology Certified Nurse (OCN) preferred
• Three (3) years or more of experience as an RN
• Two (2) years or more of oncology experience required
