Job Summary

Working remotely, the full-time Florida Licensed Patient Care Coordinator will manage individualized patient care plans, coordinate care services, and ensure effective communication among clinicians, staff, and patients in the Orlando, FL area.

Key responsibilities:
  • Complete individualized patient care plans and perform care management using electronic medical records
  • Maintain frequent contact with patients to provide support and manage compliance with care management programs
  • Document all client communications accurately and keep the Team Supervisor informed of care plan issues
Required qualifications:
  • Compassionate and empathetic with a strong commitment to customer service
  • Proficient in computer use, including Microsoft Office programs
  • Knowledge of basic healthcare terms and principles
  • Ability to multitask and remain organized in a fast-paced environment
  • Must pass a drug screen and background check
