Job Summary

A company is looking for an LPN Patient Navigator.

Key Responsibilities

• Assist patients and families in coordinating care and managing health needs
• Support the RN in managing patient caseloads and addressing clinical inquiries
• Identify and address barriers to care to optimize patient outcomes

Required Qualifications, Training, and Education

• Valid Licensed Practical Nurse multistate or Florida single-state licensure
• Three years of LPN experience; oncology experience preferred
• Strong organizational skills and ability to prioritize tasks
• Ability to work autonomously and in a remote team environment
• Proficiency in Microsoft Word, Excel, and Outlook
