Job Summary

Supporting a patient panel in the Florida market, the full-time Florida Licensed Registered Nurse will coordinate patient-centered care, manage clinical support, and ensure effective communication and documentation while working remotely.

Key responsibilities

• Coordinate patient-centered care by partnering with providers and care teams to ensure seamless care delivery
• Collaborate with care team members to develop and communicate care plans, addressing patient needs and goals
• Manage provider schedules and maintain accurate documentation of patient encounters and clinical interventions

Required qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of Florida
• Associate's Degree in Nursing (ADN) or higher
• 2+ years of clinical nursing experience, preferably in primary care or related settings
• Demonstrated ability to perform clinical triage and manage chronic conditions
• Proficiency with electronic health records (EHRs) and clinical documentation standards
