Florida Licensed RN Care Coordinator

Job Summary

A company is looking for an RN Care Coordinator to work remotely, providing patient-first healthcare and managing care for high-risk patients.

Key Responsibilities

• Collaborate with a multidisciplinary care team to develop goals and plan interventions for high-risk patients
• Coordinate care and disease management, including conducting comprehensive clinical assessments
• Serve as a liaison for patients and families, assisting with transitions of care and connecting to community resources

Required Qualifications

• Graduate from an accredited School of Professional Nursing
• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification through the American Heart Association (AHA)
• Three to five years of nursing experience
• Preferred: Bachelor's of Science in Nursing (BSN) and specialty certification
