Key Responsibilities

• Assess and evaluate members' needs to achieve and maintain wellness
• Guide members and families in accessing resources for appropriate care
• Collaborate with case managers and leadership to reduce hospitalization risks

Required Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of FL
• Bachelor's degree
• 3 - 5 years of clinical home health experience
• Experience in telephonic assessments and care coordination
• Strong collaboration skills with interdisciplinary teams
