Florida Licensed RN Care Manager

Job Summary

To support effective hospital resource management, the full-time Florida Licensed RN Care Manager will assess patients for care coordination, manage utilization and transition planning, and ensure compliance with regulatory standards while working remotely after an initial three-month onsite period.

Key responsibilities

• Conducts initial and concurrent assessments of patients to determine care coordination needs and monitors length of stay
• Facilitates multi-disciplinary patient care conferences and manages disputes while communicating care plans to patients and families
• Ensures adherence to regulatory standards and the Utilization Management Plan for effective patient care transitions

Required qualifications

• Graduate of an approved school of nursing
• Current Florida RN license is required
• Three years of experience in chronic disease management, care management, or utilization management
• Handle with Care Certification required for the Behavioral Health Unit within 90 days of hire
• Experience in acute clinical care is preferred
