Florida Licensed RN Case Manager

Job Summary

To support the Integrated Kidney Care National Flex Team, the full-time Florida Licensed RN Case Manager will autonomously coordinate quality care for ESRD and CKD patients, collaborating with healthcare teams while working remotely to minimize fragmentation in care delivery.

Key responsibilities

• Coordinate and facilitate care among patients, families, physicians, and care teams to ensure effective healthcare delivery
• Manage a portfolio of patients, addressing their needs and coordinating services across the healthcare system
• Utilize clinical data to drive outcomes related to patient satisfaction, clinical effectiveness, and financial performance

Required qualifications

• A.D.N. degree from an accredited nursing school required; B.S.N. preferred
• Minimum of 1 year of experience as an IKC RN Case Manager and 5 years in clinical nursing required
• Compact Nursing License highly preferred
• Current CPR certification and continuing education credits as required by state of practice
• Demonstrated proficiency with clinical software programs, including Capella, within 90 days of employment
