Florida Licensed RN Case Manager

Job Summary

Working remotely from the Jacksonville area, the full-time Florida Licensed RN Case Manager will manage care transitions for maternity patients, conduct health assessments via calls and home visits, and coordinate services to reduce hospitalizations and enhance patient education.

Key responsibilities:

• Making outbound calls to assess members' health status and conducting home visits as needed
• Coordinating care and making referrals to outside sources while educating members on disease processes
• Documenting interactions and tracking findings to ensure members receive necessary services post-discharge

Required qualifications:

• Current and unrestricted Registered Nurse license in the state of Florida
• 3+ years of experience in a hospital setting, specifically in OB and/or Maternity care
• 1+ years of experience with MS Word, Teams, Excel, and Outlook
• Residence in the Jacksonville metro area
