Florida Licensed Social Worker

Job Summary

Supporting the Chronic Care Management program, the fully remote Florida Licensed Social Worker will conduct psychosocial assessments, collaborate with care teams on individualized plans, and provide resource navigation for patients with chronic conditions.

Key Responsibilities

• Conduct psychosocial assessments to identify patient needs and barriers to care
• Collaborate with interdisciplinary care teams to develop and execute individualized care plans
• Provide short-term supportive counseling and facilitate access to community-based resources

Required Qualifications

• Active Florida Social Work License; LCSW preferred
• Bachelor's or Master's degree in Social Work (MSW strongly preferred)
• 2-3 years of experience in care management, medical social work, or chronic disease management
• Strong understanding of social determinants of health and care coordination principles
• Ability to work independently and manage caseloads effectively
