Florida Licensed Social Worker

Job Summary

To support patient care in a remote/hybrid setting, the full-time Florida Licensed Social Worker will assess the psycho/social needs of patients and families, educate them on community resources, and coordinate assistance for various needs such as food, shelter, and healthcare costs.

Key responsibilities

• Assess patients' situations by conducting interviews and gathering medical, psychological, and social information
• Research and connect patients with community resources, including assistance for pharmaceutical costs
• Facilitate patient placements to Skilled Nursing Facilities and support care plan coordination with employees

Required qualifications

• Bachelor's degree in Social Work; preference for a Master's in Social Work
• Proficiency in using Athena Database software, Microsoft Excel, and Word
• Ability to interpret complex documents and respond to sensitive inquiries
• Experience in assessing and addressing social determinants of health
• Strong interpersonal skills for effective communication with patients and community resources
