Job Summary

A company is looking for a Telehealth Provider (PA, CNS, NP).

Key Responsibilities

Conduct telephonic annual wellness visits
Identify patient needs, provide health education, and offer lifestyle recommendations
Create and review patient care plans for Chronic Care Management (CCM)

Required Qualifications

Licensed as a CNS, NP, or PA in Florida
Willingness to obtain licensure in additional states
Experience in telehealth, wellness visits, and preventative care
Compliance with CEU and licensure requirements
Comfortable using electronic health record (EHR) systems and medical software
