Job Summary

A company is looking for a Telehealth Provider to conduct remote wellness assessments and care plans for patients.

Key Responsibilities

• Conduct telephonic annual wellness visits
• Create patient care plans and document patient interactions
• Collaborate with the care team to ensure appropriate patient referrals and care

Required Qualifications

• Licensed as a Physician Assistant, Clinical Nurse Specialist, or Nurse Practitioner in Florida
• Experience in telehealth, wellness visits, and preventative care
• Willingness to obtain licensure in additional states
• Compliance with CEU requirements for licensure
• Familiarity with electronic health record (EHR) systems and medical software
