Florida Licensed Telehealth RN

Job Summary

A company is looking for a Telehealth RN to serve as a Care Manager for geriatric behavioral health.

Key Responsibilities

• Provide a minimum of 60 minutes of patient and/or caregiver touchpoints per month through telehealth or phone-based care management
• Monitor mental health symptoms, cognitive and behavioral changes, and medication adherence
• Support dementia care planning and coordinate services within a multidisciplinary team

Required Qualifications

• Active, unrestricted Florida RN license
• Experience in geriatric care, behavioral health, or care management strongly preferred
• Strong understanding of mental health conditions in older adults
• Experience supporting caregivers and families
• Comfortable delivering care via telehealth and phone-based outreach
