Job Summary
Providing telephonic care and support, the full-time Florida Licensed Telephonic Nurse Practitioner will manage patient care coordination, conduct assessments, and ensure quality documentation while working remotely with a commitment to 24/7 coverage, including evenings, nights, and rotating weekends.
Key responsibilities:
- Coordinate and manage care for members through telephonic and video platforms, ensuring effective communication with caregivers and healthcare providers
- Deliver quality acute care and perform comprehensive assessments, documenting findings in compliance with regulations
- Facilitate care transitions and adjust care plans based on patient needs, while engaging in advanced care planning discussions
Required qualifications:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in Florida
- Board certification through a national board as a Nurse Practitioner in one of the specified specialties
- 3+ years of clinical experience as an Advanced Practice Clinician (APC)
- Proficient computer skills for documenting medical information in electronic medical records
