Florida Licensed Transitional Care Nurse

Job Summary

Providing telephonic outreach and support, the full-time Florida Licensed Transitional Care Nurse will assist patients recently discharged from hospitals or Skilled Nursing Facilities, ensuring adherence to treatment regimens and facilitating care coordination, all while working remotely.

Key responsibilities

• Conduct telephonic assessments and provide education on treatment adherence and medication management
• Schedule face-to-face visits with Primary Care Providers and arrange specialist appointments as needed
• Identify community resources and facilitate access to necessary care and services for patients and families

Required qualifications

• Completion of an accredited LPN program
• Current Florida LPN License preferred
• Two years of related experience and/or training, or equivalent combination of education and experience
• Ability to navigate hospital EMR systems effectively
• Experience in patient outreach and care coordination
