Job Summary

Working remotely on weekends, the full-time Weekend Virtual Care LPN will conduct virtual patient visits, perform triage, and support chronic disease management while ensuring high-risk patients receive proactive care from home.

Key responsibilities

• Conduct virtual patient visits and clinical assessments
• Perform triage and identify early warning signs or risk trends
• Monitor telehealth data and escalate concerns appropriately

Required qualifications

• Active, unencumbered Florida LPN license
• Minimum of 2 years of home health experience
• Experience with triage and hospitalization prevention
• Strong clinical judgment and communication skills
• Comfortable using telehealth platforms and EMR systems
