Job Summary

Working remotely every weekend, the full-time Weekend Virtual Care Nurse (LPN) will conduct virtual patient visits and clinical assessments, focusing on reducing hospitalizations and improving patient outcomes through proactive support and intervention.

Key responsibilities

• Conduct virtual patient visits and clinical assessments
• Perform triage and identify early warning signs or risk trends
• Support chronic disease management and patient education

Required qualifications

• Active, unencumbered Florida LPN license
• Minimum of 2 years of home health experience
• Experience with triage and hospitalization prevention
• Comfortable using telehealth platforms and EMR systems
• Reliable internet connection and private remote workspace
