Key Responsibilities

• Conduct virtual patient visits and clinical assessments
• Provide education on chronic disease management and monitor telehealth data
• Perform triage and early intervention to prevent rehospitalization

Required Qualifications, Training, and Education

• Active, unencumbered Florida LPN license
• 2+ years of home health experience
• Strong background in triage and hospitalization prevention
• Experience with high-risk or complex patients preferred
• Comfortable with telehealth technology and documentation systems
