Florida RN Case Manager

Job Summary

A company is looking for an RN Case Manager in Florida.

Key Responsibilities

• Coordinate all aspects of patient care, including medical, behavioral, functional, and social needs
• Conduct intake calls, ongoing care coordination, and needs assessments for complex patients
• Collaborate with a multidisciplinary team to address care gaps and support patients during care transitions

Required Qualifications

• Current RN License in good standing in the state of practice
• Bachelor's degree in nursing or an associate's in nursing with another clinical or business bachelor's degree
• Minimum of 4 years of clinical experience, with at least 2 years in case management
• Certified Case Manager (CCM) certification or commitment to obtain when eligible
• Geriatric care experience is highly desired
