Florida RN Case Manager

Job Summary

To support a growing practice, the full-time Florida RN Case Manager will coordinate all aspects of patient care, advocate for patients, and engage in ongoing care management while working remotely.

Key responsibilities

• Conduct initial intake calls and ongoing patient outreach to review and update care plans
• Provide continuous care coordination for a panel of complex patients, ensuring their needs are met
• Collaborate with a multidisciplinary team to address care gaps and support patients during transitions

Required qualifications

• Current RN License in good standing in the state of Florida
• Bachelor's degree in nursing or an associate's degree in nursing with a clinical/business bachelor's degree
• Minimum of 4 years of clinical experience, with at least 2 years in case management
• Certified Case Manager (CCM) certification or commitment to obtain it when eligible
• Geriatric care experience is highly desired
