Florida RN Case Manager

Job Summary

To support a growing population health team, the full-time Florida RN Case Manager will coordinate comprehensive patient care, advocate for patients, and engage with a multidisciplinary team while working remotely.

Key responsibilities

• Conduct initial intake calls and ongoing patient outreach to update care plans and monitor progress
• Provide care coordination for a panel of complex patients and develop plans of care in collaboration with the care team
• Support patients during care transitions and actively participate in interdisciplinary team meetings

Required qualifications

• Current RN License in good standing in Florida
• Bachelor's degree in nursing or an associate's in nursing with another bachelor's degree
• Minimum of 4 years of clinical experience, including at least 2 years in case management
• Certified Case Manager (CCM) certification or commitment to obtain when eligible
• Geriatric care experience is highly desired
