Job Summary

To support patients recently discharged from the hospital, the full-time Florida Transitional Care Coordinator will make outbound calls to educate patients about the Home Visit Program, encourage their participation, and schedule appointments while working remotely.

Key responsibilities
  • Make outbound calls to introduce and educate patients about the Home Visit Program
  • Build rapport with patients and encourage participation in transitional care services
  • Manage patient information and documentation within healthcare systems
Required qualifications
  • Minimum of 1 year of experience scheduling patient appointments
  • Minimum of 1 year of experience in a healthcare setting
  • Previous experience in sales, customer service, call center, or healthcare coordination preferred
  • High School Diploma or GED required
  • Strong computer skills and ability to navigate multiple software systems simultaneously
