Job Summary
To support patients recently discharged from the hospital, the full-time Florida Transitional Care Coordinator will make outbound calls to educate patients about the Home Visit Program, encourage their participation, and schedule appointments while working remotely.
Key responsibilities
- Make outbound calls to introduce and educate patients about the Home Visit Program
- Build rapport with patients and encourage participation in transitional care services
- Manage patient information and documentation within healthcare systems
Required qualifications
- Minimum of 1 year of experience scheduling patient appointments
- Minimum of 1 year of experience in a healthcare setting
- Previous experience in sales, customer service, call center, or healthcare coordination preferred
- High School Diploma or GED required
- Strong computer skills and ability to navigate multiple software systems simultaneously
