Job Summary

GA, PA, OH Licensed Advanced Practice Provider, responsible for delivering comprehensive care to complex patients through telehealth while collaborating with an interdisciplinary team, in a fully remote, full-time position.

Key Responsibilities

• Deliver exceptional care to complex patients using video telehealth visits
• Guide medical management and coordinate care with interdisciplinary teams and patient support systems
• Perform comprehensive assessments, medication reviews, and maintain accurate patient records

Required Qualifications

• Active RN and APRN or Physician Assistant license in GA, PA, and/or OH
• Three or more years of experience as an APP in relevant fields such as primary care or geriatrics
• Proficiency in telehealth technology and electronic health records (EHR)
• Active BLS certification at the time of hire
