GA, PA, OH Licensed Advanced Practice Provider

Job Summary

To support a multidisciplinary team, the fully remote Advanced Practice Provider (APP) will manage care for complex patients in GA, PA, and OH, focusing on symptom management, chronic disease management, and collaboration with care teams to deliver comprehensive patient support.

Key responsibilities

• Deliver exceptional care to complex patients through video telehealth visits and interdisciplinary collaboration
• Guide medical management of patients using the 5M framework and perform comprehensive assessments
• Maintain accurate patient medical records and ensure compliance with legal and ethical guidelines

Required qualifications

• An active and clear RN and APRN or Physician Assistant license in GA, PA, and/or OH at the time of hire
• Three or more years of experience as an APP in primary care, geriatrics, or palliative care
• Proficiency in telehealth technology and electronic health records (EHR)
• Active BLS certification at the time of hire
