Job Summary

A company is looking for a Breast Nurse Navigator.

Key Responsibilities

• Identify and engage patients for the Comprehensive Breast Health Program and coordinate care across services
• Monitor remote patient data and facilitate communication between patients and healthcare providers
• Serve as a clinical resource and participate in educational offerings and performance improvement projects

Required Qualifications

• Bachelor's degree in Nursing or an accredited Nursing program
• Current RN license (Single State or Multi-state Compact)
• BLS certification required upon hire
• Minimum 3 years of adult acute care clinical experience, with 2 years in women's or breast health preferred
• Experience in case management and/or remote patient management preferred
