Key Responsibilities

• Engage and educate caregivers via phone and digital technology to empower them in managing care
• Assess caregiver and care recipient needs and assist in creating care plan goals
• Document coaching activities and collaborate with stakeholders to improve care plans and outcomes

Required Qualifications

• 4+ years of experience in care planning or case management
• Experience working with the elderly or adults with disabilities
• Remote case management experience is highly desired
• Comprehensive knowledge of industry concepts and principles
• Proven experience accessing community resources and navigating mobile applications
