Job Summary

Managing care coordination activities for assigned members, the full-time Georgia Licensed Care Coordination Manager will oversee team members, ensure continuity of care, and address escalated issues while working remotely with occasional in-office meetings.

Key responsibilities

• Monitor and review member support care coordination activities to ensure appropriate service delivery and continuity of care
• Oversee and approve assigned caseloads based on state and contractual requirements, as well as member needs
• Provide guidance and resources to the care coordination team to address member and provider issues effectively

Required qualifications

• Bachelor's degree required
• 5+ years of related experience in care coordination or a similar field
• Demonstrated leadership and team development skills
• Strong call center experience preferred
• Experience with compliance and regulatory guidelines in care coordination
