Job Summary

A company is looking for a Care Coordinator Nurse I- AWV (LPN).

Key Responsibilities

• Manage inbound and outbound patient interactions to complete Medicare Annual Wellness Visit forms
• Utilize customer systems to identify upcoming AWV appointments and assist with patient education
• Ensure compliance with privacy standards and complete documentation as required

Required Qualifications

• Licensed Practical Nurse (LPN) license required, preferably in Georgia or a compact multi-state license
• Minimum 3 years of experience in a doctor's office or hospital setting
• High school diploma required; some college preferred
• Experience in the healthcare field and familiarity with medical terminology
• Typing speed of at least 55 wpm is preferred
