Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop and collaborate on ongoing care plans with providers to address member needs
• Provide education and resources to members and families regarding healthcare options and services

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• RN - Registered Nurse State Licensure and/or Compact State Licensure required
• Experience in developing personalized care plans
• Knowledge of social determinants of health and community resources
